More than 100 health organisations across Victoria are taking extra steps to improve their community’s wellbeing by delivering the Life! program.
Life! providers commit their staff and resources to raising community awareness of type 2 diabetes, heart disease and stroke prevention, as well as delivering the Life! program.
Delivering the program Life! providers:
Once an organisation is a registered Life! provider, eligible health professionals within the organisation can apply to deliver the Life! program. Find out how your health professionals can become a Life! facilitator.

More than 150 health professionals across Victoria are taking extra steps to improve their community’s wellbeing by delivering the Life! program.
Our Life! facilitators come from a variety of allied health backgrounds and include exercise physiologists, physiotherapists, nurses (div 1 and 2), dieticians and other eligible professions. To become a Life! facilitator, health professionals need to have:
Health professionals must have an affiliation with a Life! provider (organisation), or be registered as a private provider with a supporting ABN number.
We are now taking expressions of interest from health professionals to be trained to deliver the Life! program group course. The Life! program group course includes 7 sessions with participants over 12 months to support them to achieve their health goals. In Metro Victoria group courses are delivered fully online due to COVID-19. In rural and regional locations facilitators may deliver the program face-to-face providing Diabetes Victoria’s requirements for face-to-face delivery have been met.
We accept applications from registered health professionals from a range of backgrounds, including dietitians, nurses (Div. 1), exercise physiologists and physiotherapists amongst others. Applicants will be required to have completed core training with Health Change Australia. Relevant experience and skills in group facilitation applied behaviour change and motivational interviewing would be highly regarded.
Health professionals across Victoria are helping to prevent type 2 diabetes, heart disease and stroke by delivering our Life! program through a flexible Telephone Health Coaching service.
The Life! Telephone Health Coaching Service gives participants an opportunity to complete the program at a time and location that suits them.
