Attach a blood pathology report to the referral within 12 months of referral date:

Tools are available through POLAR and PenCS to assist in identifying patients both at the point of care and through clinical summary reports. These tools streamline preventative care by providing color-coded urgency alerts, helping GPs and clinic teams quickly identify patients eligible for Life!. They also enhance workflow efficiency, simplifying risk assessment and referrals.
